TEAM REGISTRATION
To assure you get the kids you want on your roster and to help Pony administer the league more efficiently we are
asking teams to register as a whole. The coach is responsible for collecting all of his player’s registrations and fee’
s and turning them in together with the Team registration form provided.
Please include the following in your packet:
1. Each player's registration and fee's.
2. The team registration form filled out.
3. Your $100.00 Team sponsor fee.(Your sponsor's logo will go on www.spokanepony.org/sponsors)
4. Managers/Coaches Application filled out.
5. Coaches/Managers Code of Conduct Contract.
6. Washington State Patrol form filled out.
Note: You must have forms 4, 5&6 for each Coach and Assistant Coaches.
